Persistent Genital Arousal Disorder (PGAD) is a condition characterized by excessive genital arousal in the absence of sexual desire or pleasure. PGAD can be due to an increase in blood flow to the genital area, causing the clitoris or penis to swell. This engorgement may cause increased pressure on the pelvic nerves resulting in tingling, throbbing and itching in the genital region. Specifically with PGAD, the pudendal nerve is irritated within the pelvic girdle. This nerve supplies sensory, autonomic and motor control that carries signals to and from the genitals, anal area and urethra. Often the pudendal nerve can become irritated due to hyperactive pelvic floor muscles, poor hip or spinal mobility and poor movement patterns. Your physical therapist can help alleviate tension in the involved muscles and tissues using manual therapy techniques as well as provide you with appropriate exercises and stretches to help lessen the load on your pelvic floor.
